Purpose of position: To be part of the multidisciplinary team meeting the needs of the service user in ways that values the dignity of the individual and promoting choice, respect, participation and independence. Care provided by community support workers is expected to include care that would responsibly be given by members of the service users own family. Any clinical experience required will be provided by relevant others or full training will be given to undertake care requirements.

 

Job role and principle responsibilities:

1.     To assist service users with their personal care needs, including assistance with dressing, washing, bathing and toileting.

2.     To assist service users with mobility and other physical needs, including continence and assistance with the use and care of aids/adaptations and all personal equipment.

3.     To care for service users who are temporarily sick and require support with various non-nursing duties or clinical duties with specialist training and competency sign off.

4.     To help care for service users with Palliative or End of life diagnosis.

5.     To undertake housekeeping and food preparation duties when need arises.

6.     To support and promote good standards of food and hygiene, social skills and independence.

7.     To help in the promotion of physical and emotional stimulus for the individual service user through positive interaction, ensuring the opportunities for accessing the community are met. Promote activities such as reading, writing, hobbies and recreations.

8.     To communicate effectively with the service users using chosen methods of communicating that service users prefer to use. Promoting uniqueness and individuality.

9.     Keep accurate records and report on service provided. Record and report concerns about or for service users and attend and record meetings with others in the multidisciplinary team. Report to clinical manager any significant changes in health or circumstance of service user.

10.   Attend all relevant training required for job role.

11.   To be aware of, comply, sign and read policies and procedures for H and H healthcare.

12.   To promote and encourage services users to remain as independent as possible.

13.   Respect and adhere to H and H healthcare philosophy of care.

14.   Be aware of and comply with services smoking policy, dress code, policy use of mobile phones and code of conduct and policy on drugs and alcohol.

15.   To be aware of medication policies, procedures assessments, and accurately keep records and report. To administer medication being aware of protocols and completing medication administration charts.

16.   To value each service user and other employee as an individual. Promote their dignity, self-esteem, independence, choice, respect and human rights.

17.   To carry relevant paperwork and replace or replenish in service user’s files.

Health and safety

To carry out duties in line with safe working practises ensuring adherence to the health and safety standards e.g. safe manual handling practises.

To use all equipment provided when providing care e.g. hoists and other manual handling aids as dictated in the client’s individual risk assessments. Check all equipment has been serviced and is up to date and in service.

1.     To monitor work areas and practises to ensure they are safe and free from hazards.

2.     To apply infection control and food hygiene measures within the service users home environment.

3.     To be aware of PPE regulations and wear protective clothing and gloves.

4.     To attend any training deemed necessary and appropriate for the job role.

5.     To notify the on-call coordinator of any changes or difficulties out of normal office working hours using the emergency number (purple Phone).

Learning and development

1.     To understand the scope of the role and identify how this may develop over time.

2.     To assist with the inducting, coaching and mentoring of new employees including shadow shifts.

3.     To identify personal training and development needs to participate in the staff personal development programme.

4.     To participate in team meetings, supervisions and appraisals and relevant training

5.     To maintain a staff folder with policies and procedures and any handout from training and be familiar with the content.

6.     Disseminate learning and information to other team members to share good practise and inform others about current and future developments.

Essential experience

1.The ability to work on own initiative and without basic supervision

2. The confidence to liase and work with other professionals and agencies, ie, doctors, district nurses, physiotherapists etc.

3. The drive and desire to promote and enable a client’s independence

 

Desirable

1. Experience of domiciliary care obtained either in employees own household, in voluntary work or in paid employment

2. Experience of working 1 to 1 with elderly or dependent clients

3. familiarity with the needs of the service user or knowledge of illness/diagnosis

4. The ability to relate positively to dependent clients and other clients and other carers
